FARMINGTON 500 WAIVER

-THIS MUST BE SIGNED AND RETURNED –

I/We the parent(s) or guardian(s) of the above named child hereby authorize and approve the child’s participation in any and all supervised Farmington 500 activities during the current season and agree to furnish a certified birth certificate for the child upon request of league officials.

I/We assume all risks and hazards incidental to the child’s participation and supervised Farmington 500 activities including, without limitation, any and all risks and hazards arising out of transportation to and from such activities.

I/We willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

I/We acknowledge that I am aware that there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from: an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respirator syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.

I/We do hereby release, acquit and discharge the Farmington 500 and the organizers, sponsors, supervisors, participants and persons transporting the child to or from such activities from any claims, demands, actions, causes of action, damages, costs, loss of services, expenses and compensation on account of or in any way arising out of such activities except to the extent that it may be covered by accident or liability insurance.

I/We do hereby agree to return, upon request or within thirty (30) days of the end of the current season (whichever occurs first), the uniform and other equipment issued to the child in as good condition as when received except the normal wear and tear. Upon failure to return the uniform or other equipment as provided above, I/We agree to be responsible for the cost of replacement for the uniform/equipment.

I/We understand there are no dogs allowed and no smoking, smokeless tobacco use, e-cigarettes, vapor use, chewing tobacco or any other product that might be considered smoking allowed anywhere on the Farmington 500 property and doing so will result in potentially being asked to leave the property.

I/We agree to the terms listed above and also to participate in league fundraisers, tag day events and will work the concession stand when asked by my/our child’s team mom… Failure to participate will be a game suspension.
